How to soothe back and leg pains

As you wind up the second trimester, you may start noticing some new symptoms, such as backaches. About half of women have back pain at some point during pregnancy, usually in the lower back. You may have lumbar pain, which is felt around your spine at about waist level. It may be aggravated by sitting for long periods of time, standing, or lifting — and it tends to be worse at the end of the day. Then there's what's known as posterior pelvic pain, which you may feel deep inside your buttocks (on one side or both) or in the back of your thighs. This pain can be triggered by activities such as walking, climbing stairs, getting in or out of a low chair, twisting, and lifting. Positions in which your hips are bent — such as when you're sitting in a chair and leaning forward — may make posterior pelvic pain worse.

You may also find that your leg muscles cramp up now and then. They're carrying extra weight, after all, and your expanding uterus is putting pressure on the veins that return blood from your legs to your heart as well as on the nerves leading from your trunk to your legs. Unfortunately, the cramps may get worse as your pregnancy progresses. Leg cramps are more common at night but can also happen during the day. When a cramp strikes, stretching the calf muscle should give you some relief. Straighten your leg and then gently flex your toes back toward your shin. Walking for a few minutes or massaging your calf sometimes helps, too.

Soothing back pain

**************************

Getting regular exercise, using good body mechanics, and paying attention to your posture can help keep backaches at bay or ease pain you already have. Here are some other suggestions:

• Avoid sitting or standing for long periods of time. Take frequent breaks.

• Take it easy and don't get overly tired.

• Avoid activities like vacuuming and mopping that require you to bend and twist your body at the same time. (If there's no one else to do these chores, move your whole body rather than twisting or overreaching to get to out-of-the-way spots.)

• Soak in a warm bath or use a hot pack on the affected area. (Or try a cold pack if heat doesn't work for you.)

• Ask your partner to rub your back, or schedule a prenatal massage with a trained therapist.

• Sleep on your side with one or both knees bent and a pillow between your legs and another pillow (or a wedge) to
support your abdomen.

Above all, always listen to your body, avoiding particular activities or exercises that make your back ache.

Diarrhea During Pregnancy

Constipation is a common side effect of pregnancy. But what of diarrhea? No pregnant woman wants to suffer from diarrhea during pregnancy. Unfortunately many do. Diarrhea may result from a number of causes during pregnancy. The more common culprits include a lactose intolerance, dietary or hormonal changes or sensitivities to certain foods while pregnant.

Fortunately diarrhea during pregnancy is typically short lived and not something to cause concern. If however you experience diarrhea accompanied by severe nausea or vomiting, it is important you consult with your healthcare provider. You may need medical intervention to help prevent dehydration.

Diarrhea in Late Pregnancy
Many women report having diarrhea right before they start labor. This may be the body's way of preparing room for delivery. Diarrhea that is severe may also result from viruses or food poisoning. Some women find that sugar free candies or other artificial products can contribute to bowel problems in late pregnancy. If you find you suffer from chronic diarrhea during pregnancy you may want to re-evaluate your diet.

Some women suffer from diarrhea for a period of time only to suddenly develop watery diarrhea. While unpleasant, this is often the body's way of readjusting itself properly. Chronic constipation can result in a condition called fecal impaction, where the stool partially obstructs the bowels. Typically women experiencing this have other symptoms like nausea, vomiting or abdominal bloating. You may need to seek the advice of your caregiver if this is the case.

You can suspect food poisoning as the culprit if your diarrhea comes on suddenly and violently but clears up within a 24 hour time frame. Pregnant women are also more susceptible to stomach flu and related disorders during pregnancy.

Treating Diarrhea During Pregnancy
If you do come down with diarrhea during pregnancy you'll want to make sure you remain hydrated. Be sure to drink plenty of water or hydrating fluids like Gatorade. Bland foods that can help bind and resolve mild cases of diarrhea include rice, toast or bananas. Be careful you don't consume too much of these products however, as you might find yourself constipated in a few days.

If you ever experience diarrhea accompanied by pain or fever you should be sure to contact your healthcare provider immediately. The same is true if you notice that your stool contains mucous or blood in it. This is a sign that something more complicated is happening that may require treatment.

Preventing Diarrhea During Pregnancy
You can prevent constipation related diarrhea from impacted bowels by eating a diet that is well balanced and contains plenty of fiber. Many women find they are constipated during pregnancy in part because of their prenatal vitamins, which contain high levels of iron that is often binding.

Foods rich in fiber include fruits and vegetables. Some women find it helpful to drink prune juice or eat plenty of salads during pregnancy, which can help alleviate constipation during pregnancy. You should also be sure to drink plenty of water during pregnancy, which will help keep things regular and flowing. Constipation often results from dehydration or too little fluid intake during pregnancy. Be sure to watch your intake of caffeinated beverages including pop or coffee, which can contribute to dehydration. Eating foods high in fiber and drinking plenty of water will also help reduce any side effects of constipation, which may include hemorrhoids during pregnancy.

Anemia and the Pregnant Woman

Iron deficiency occurs in 20% of pregnant women in developed countries. Even worse, 50% or more of women in nonindustrialized nations become iron deficient and between 30% and 50% are deficient in folic acid. Severe anemia is associated with a higher mortality rate among pregnant women. Mild to moderate anemia, however, does not pose any elevated risk.

Pregnancy increases the risk for anemia in different ways:

  • Pregnancy increases the body's demand for folic acid and, therefore, poses a risk for deficiencies and an increased risk for megaloblastic anemia. Low levels of folate during pregnancy increase the risk of neural tube defects in newborns.
  • Pregnancy also increases the demand for iron, thus posing a risk for iron deficiency anemia. Pregnant or nursing women require 30 mg of iron per day. Maternal iron deficiency anemia is associated with increased weight or size of the placenta, a condition that may pose a risk for later high blood pressure in the offspring. Pregnant women with low hemoglobin levels (the iron-bearing component in the blood) have an elevated risk for pre-term or low birth weight infants. (However, iron supplements do not appear to have any effect on these complications.)
  • Pregnancy is also associated with fluid retention, which in turn may produce high volumes of plasma (the fluid component of blood). This can dilute red blood cells, which may lead to anemia.
  • After delivery, heavy bleeding, which occurs in 5% to 10% of women who have given birth, can cause symptoms of anemia.

Diagnosing of Iron Deficiency During Pregnancy

A diagnosis of iron deficiency is problematic in pregnant women. The standard test is a measurement of ferritin levels, which are low in most people with iron deficiency. Pregnant women, however, may have high ferritin blood levels into their third trimester but still be iron deficient. A newer test that measures a factor called serum transferrin receptor may prove to be a useful way of diagnosing iron deficiency in women.

Preventing Anemia in Pregnant Women

Iron Supplements. For the past 40 years, iron supplements have been recommended for all pregnant women. This practice has been challenged recently, however. There is no clear-cut evidence that the mild iron deficiency in most pregnant women experience is harmful. In addition, iron supplement causes gastrointestinal side effects and may not be completely harmless. On the other hand, a 2003 study reported that women who took iron supplements had children with higher birth weights than those who received placebo. (Iron pills had no affect on anemia, however.)

Some experts suggest iron supplements for the following women:

  • All pregnant women whose hemoglobin levels are less than 11 g/dl, and
  • Pregnant women whose serum ferritin levels are low beginning in their 20th to 24th weeks of pregnancy.

Vitamin Supplements. Women who are trying to conceive, who are pregnant, and who are breastfeeding should take 400 mcg of folic acid a day. (They should be sure this is folic acid and not folate, which is the natural form -- but supplements at the same dose are half as potent.)

Pregnant and nursing women who are vegetarians should be sure to have supplements of folic acid and other B vitamins as well, since many of these nutrients are found primarily in animal products. Of particularly importance, vitamin B12 deficiencies during pregnancy can also produce anemia in both mother and child.

Diets Rich in Vitamin C. Eating foods rich in vitamin C can help absorb iron.

Treating Anemia During Pregnancy

Pregnant women who become anemic and require treatment may be given oral iron supplements or transfusions in severe cases. Intravenous iron sucrose is a newer form of IV iron and may be prove to be effective and safe for pregnant women with anemia.
